Australian star Steve Smith is in concussion protocols after not returning to the field in the first Test against South Africa.

The Australian camp revealed the news in the final session of day four after Smith failed to join his teammates in the field.

The 37-year-old had an incredible day three in Durban, turning the match with a trio of spectacular catches.

However, day four didn’t go in his favour as he took an Anrich Nortje short ball to the helmet on the way to holing out for 27.

Nortje is the fastest bowler in the contest and recorded the fastest speed of the match on day four at 150.1km/h.

Smith looked fine in the moments after the blow in the first session of play and was cleared to bat on by Australian medical staff.

However, after Australia declared at the end of the second session, having set the home side 421 for victory, all-rounder Michael Neser was seen warming up during the Tea break.

He then took to the field as play resumed, fielding in the gully region.

The 37-year-old also appeared to hurt his finger taking a catch off Beau Webster on Sunday that closed the Proteas’ first innings.

A concussion could put his involvement in the second Test, which starts on Sunday, in jeopardy.

An update is expected on Tuesday.

Smith has had trouble with Vertigo in the past that affects his vision and balance.
